Going anonymous on this one, as the industry is competitive in Indonesia, and I'm a foreigner. Don't know if my boss, or my colleagues are on Quora, so I need to be careful.
I'm on 20,000,000 Rp ($1475US) per month, as a consultant. I'm not even in a dedicated managerial role, and I spend most of my time at home. I live in a smaller city, in the centre of Java, where most things are considerably cheaper, than in Jakarta, or Bali.
My work consists of design, consultancy, and manufacturing, and my latest project, will help small, remote farming villages, all over Indonesia, empowering them to increase harvests and reduce wastage, thus increasing their profits, which should provide a slightly higher standard of living for them.
I do love my job, and it's the most relaxed job I've ever had. My boss isn't the easiest of people to work with, but he is one of the most hospitable boss’, I've ever had.
It's not the highest salary I've received, as I took a 50% pay cut, in my birth country.
But the salary in Indonesia, puts me in a higher class, than in my birth country. I was in the lower of middle class back there, and this salary puts me into the higher part of middle class, in Indonesia.

I live comfortably, in a place where poor people live around me. I pay an old guy 100,000 Rp, once a week, to clean my garden, and take my trash. I basically give him 2 days salary, for 2 hours work.
My mother built a house, but resides in my birth country, so I have the run of a two-story, three bedroom house, which is quite large, in comparison to her house, back in my home town. The house here, has many modern conveniences, such as sit down toilets, high power input (2,400w), air conditioning in all the bedrooms, a 40″ LCD TV, Broadband Internet, Microwave, Chilled and Hot water dispenser, and top-loading washing machine - considered basics, in my birth country.
I don't pay rent, just electricity, community security, and land tax.
It also make me sad, when I found out my indonesian colleague, only earns 12,000,000 Rp, and has a wife and three children to feed. Although, I do feel like he is a king in his neighbourhood, where his neighbours, probably don't earn half what he does.

I’m in Turkey, working in healthcare (physician). I make approximately 6200 TL in the beginning of the year, but because of the tax system, the income tax I pay in January (15%) gradually increases to 35%. And because of the method I’m getting paid is a little complex, I think I end up with 5400 TL in the 12th month.
I mentioned the tax system, because I learnt that the football players in Turkey have 15% income tax fixed. Yep, those who make millions have lower taxes than those who barely make thousands (I’m not just talking about myself, millions of people have to be magicians and make it work with just 1600 TL/month). This makes me mad.
Okaaay… Now… Let’s round the numbers and say I make 6000 TL/month, which is 72000 TL/year. Big number, right? That’s actually 19000–19500 USD/year depending on how Turkish Lira performs.

Am I happy? No. I am vocal about this, and people often tell me that I look greedy for that. But, am I really?
It may sound so when you think of millions of people who make 1/4 of my salary, but the people who think I’m greedy do not know the details. For instance, any specialist makes the doubleof what I earn. That sounds natural, but they make that much, because I somehow am not entitled to the circulating capital even though I make significant contribution to it. Why? BecauseI could not beat the average ‘performance point’.
This is about to turn into a rant, but that’s my reason behind my unhappiness with my salary.
Yes dear Turkish reader, your physician is under pressure of scoring points while he’s trying to cure you. That is why some of the doctors get mad when you tell them they earn much - They do not. Simple as that. So what’s this ‘circulating capital’ and why is the doctor you love so much not getting anything from it? Let’s examine this situation, okay? Basically every procedure is appointed a price, and sometimes a point. For instance, my examination costs 15 TL and it has 20 points.

Case 1: I examine you, I don’t get the money, but I get the point. I order some bloodwork, they have their price, but not the point. Nurse sometimes opens an IV route while taking blood, because you might need some IV fluids. IV route has its price, but not the point. Your bloodwork indicates something is wrong with you, but it’s not definitive. So I order an ultrasound. An ultrasound has its price and its point, but the point is not added to my chart, it is added to the radiologist’s. Fair, because he’s the one who does the ultrasound. Okay moving on. He says you have appendicitis. Now that I have my diagnosis, I can give you some painkillers. An IV treatment has its price, but not points. Then I have to consult with general surgeon. From that point, all the points (if the procedure has one, of course) goes to the general surgeon as expected. Because we aren’t sure if we can beat the average of the hospital that month.

Case 2: You somehow managed to be in the right place when a rock fell from the sky and hit on your foot (THAT happened). It hurt as hell, so you go to an ER and you met me. I checked your foot. That’s 20 points for me, again. I order an X-ray (has price, no points). I examine the X-ray, found out you have a broken bone. I reduce it (reducing has points and price, but wait), then I put your foot into a splint so it doesn’t move (splint also has points and price, but again, wait). You thank me, I tell you to get it checked to an orthopedist. You leave. I go to my computer so I can document what I did to you. I put examination (20 points, yey!), X-ray (cost you money, nothing to me), bone reduction but what’s that? System told me that I can’t get points from it because I, an ER doctor, am not qualified for it! That’s fair, that’s what orthopedists are for. But system also charges you for it. You don’t pay, so neither you nor I care. I add splint (I don’t remember the exact point, but it’s something like 100), but I am warned that I’m not qualified by the system. I get points. Because I’m good at Mario Bros.

Case 3: You, again, somehow managed to cut yourself. You check it, you are not sure if it needs stitching so you come to the ER and here I am, sitting on 140 points and no cash. AGAIN, I check you (20 points to me), I decide that no imaging is needed because I don’t think anything left in the wound. I stitch your wound, dress it, and tell you to come tomorrow to renew the dressing. I prescribe an antibiotic and maybe a painkiller. You go to the pharmacy, you find out that you have to pay an examination fee and a prescxtion fee, and a fraction of the price of the drugs I prescribed. You think “That bastard got the money.” You still get the medication because you don’t want to die from an infection. Meanwhile, I received 20 points from examining you, 50 points from stitching you, but system warned me that I’m not qualified to stitch you up. YES, THE SAME SYSTEM WHICH PUT ME TO WORK IN THE ER THINKS I’M NOT QUALIFIED FOR THE SIMPLE PROCEDURES LIKE THAT. GODDAMN SYSTEM HAS NO PROBLEM LEAVING CRITICALLY ILL PEOPLE TO THE HANDS OF A ‘DOCTOR’ WHO CAN’T EVEN STITCH ACCORDING TO THE SYSTEM ITSELF. I am awarded pretty 5 points for medical dressing I just did. Lucky for all of us, I was qualified to do that so system does not warn me about it. Prescxtion doesn’t have a price or point in the system (But you PAID).

So what’s going on here? You paid money in a free healthcare system. You also have big bills to be paid by your government. You naturally think that your doctor is a greedy bastard because you goddamn PAID in a FREE healthcare system. He is THAT greedy. Meanwhile your not-so-competent but greedy doctor is left with some points. Nothing but some points. Because he did not examine patients, he played Tetris. At the end of the month, the profit of the hospital and the points the doctors accumulated by PLAYING MARIO BROS. or TETRIS - no judgement, are calculated. Majority of the profit goes to the government (and maybe to someone’s shoe boxes?). Management (not chief physician but the manager of the hospital and his staff) get their share from this profit. IF anything remains, this is to be distributed to the physicians who played Mario Bros good enough to accumulate more than the calculated average point. At this point, in order to ensure specialists get more than general practitioners, their points are multiplied by a modifier (2,4 I guess). Meanwhile general practitioners have their points multiplied by a modifier as well (1.6 I guess), because why not? Then the money is distributed but that’s also capped. Meaning after a certain point, it doesn’t even matter how much you scored, you get the same money. But you have to score more than the average. Below average gets nothing. Not even a pat on the back for the points he made. BUT THAT’S NOT IT. Because of the cap, if the hospital made big profits, sometimes there’s still money to be distributed. If you are thinking the below average physicians (The doctor who diagnosed you 3 times, and the general surgeon who operated on your appendicitis) are going to get their pats on their back, you are wrong. We forgot the most crucial part of the healthcare system, and let’s be honest, you did not notice until now: Nurses. If there is still money left, they also get their share from ‘circulating capital’, but as you can imagine it’s so little sometimes they don’t even notice if someone did not tell them. At least that’s the case for my hospital. So if anybody disrespects to any of the nurses I work with, they have a problem with me.

That’s the system. That’s the system you are diagnosed in. The people who you see the most gets paid the least. The management staff gets their share from the circulating capital no matter what. If we leave the money issues aside, there are other issues with this system, you should have noticed - they are in bold font. I am a physician who graduated from a university which the system accredited. That system trusts my university that it will graduate only competent physicians. Yet, it does not trust the person it graduated with simple procedures. So don’t be surprised when your doctor tells you he’s not qualified to remove the foreign obxt which is clearly visible from your skin, which even you can remove easily. He can do it, but the system thinks he can’t. He says he can’t, because he does not want to get into trouble by simply helping you. It doesn’t matter if you file a complaint about him removing the obxt or not. If he did, and someone notices he did, he’s potentially in trouble. Most of the times, you don’t file a complaint or a lawsuit, because you are just grateful, but the system is not. So the doctor who just risked to look like an incompetent idiot to you and all the other patients probably had trouble for doing something similar in the past, so now he just does not do it.

Back to money issues, I can hear you saying “just score more, play more Mario Bros you dumb fool”. That’s not how any of it works. The points I accumulate are audited, because system allows me to enter basically anything (it allows me to enter “liver transplant” to a patient who has a runny nose), but it warns me. You remember the procedures system warned me about? Their points are subtracted from my points (If you think they are subtracted from the bill, you are naive). And according to the system, I’m not qualified to perform basically anything that could save your life, and I work in the ER. Reducing a bone? Relocating a dislocated joint? Stitching? Removing foreign obxt from your skin, throat, ear or eye (I admit, I should not remove obxts from your eye - and some of the obxts from your throat and ear)? Intubation? Unqualified. I have done every single procedure that I knew I can, for which I was considered unqualified. I have done, am doing, and am going to keep doing those until I get in trouble for it. I am not judging a single physician for not doing those.
And if one day I get in trouble for relocating your shoulder or intubating you, I’ll never do it again. Not even if you hold a gun to my head, not even with your written consent. Because like I said, it’s not you, it’s the system.

That’s why I am unhappy about my pay and my damn job.

如果有一天我因为帮你活动肩膀或者是给你插管而遇到了麻烦,那我以后也不会再这么做了。即使你拿枪指着我的头,我也不会帮你做了。因为就像我说的,这不是你的问题,这是体制的问题。
