Most of us (especially, Indians) form a sentence in our native language, translate it in English and then use it in a conversation. You have to stop doing that. Start thinking in English. Its a bit tough, but it will become easier and spontaneous with time. Don't think in your native language, think in English. This is the biggest hurdle in the process of improving your communication skills. Once you cross it, nothing can stop you.
Now how to do that? Simple. Speak with your mind whenever you get time. But speak in English. Don't worry about grammar. Just speak.
Now, let's focus on improving your fluency.
Start reading newspapers, magazines, novels, blogs and whatever you can. But don’t just read, try to understand it. And make an opinion about it. Especially about editorials, opinions and issues.

For example, lets say you read an article about drought in India. After reading it, think about it for some time. Then talk to yourself about it. Stand in front of a mirror and explain the issue to your reflection. Sounds funny, right? But trust me, it will help. And also think about what can be done to solve the problem, its outcomes etc and tell it to your reflection. Initially you will feel embarrassed, but keep doing it and you will certainly improve your English.
One more activity that might help. I assume you have a laptop or pc (even a smartphone would work). Download some good serials (I will recommend Sherlock, Suits and GOT(caution: this one not for children)) and watch them by keeping subtitles on. Understand how they talk, how they pronounce a particular word. Then mute that serial and try to make similar pronunciations by reading those subtitles. It will not only improve your pronunciation but also boost your confidence.
One more activity that might improve your fluency in English. Find a friend/relative/anyone (being an Indian, I know that this can be a bit tough as whenever you start speaking in english, you are accused of "shininig") who speaks English fluently and converse with him as much as possible. Ask him/her to point out your mistakes and tell you how to correct them

3. Adopt a habit of answering every question in the same tense so by doing this you will get practice of speaking with the right sentence although you are using same lines like

so here your are getting the idea of how to answer the people as well as you are honing your tenses. This will give you a good start. Very soon you will have the benefit of this practice.
4. Listening is a fine art and if you understand this , it will give you enormous power to perceive how a sentence can be frxd or spoken. So you must listen. As I was telling you identify the tenses. And answer with same phrase how will you do that if you are not listening attentively.

If you really want to add the word then Don’t try to remember the words in stead of this practice it, as we all know that Rolling Stones gathers no mass so don’t force yourself to memorise 5 or 10 words daily just if you have one word and you are practicing in such a way that you can use it in your any sentence then what do you think isn’t it good as compared with having 10 word daily but don’t know how to use it.
7. Speaking to yourself. No doubts it is the best way to learn. As we all know always something or the other is roaming in our head. So if you give a particular direction to your thoughts it will be set and you will start learning effortlessly. In the beginning you have to show the path to you thoughts.
8. Get the basic knowledge of adverb adjective noun verb and with one word you would get 4 way to use it. This will give you an idea how to change the word according to sentence. If you know how to use it in different sentences then it will give you an edge over others. If you have this elementary things along with you can explore it anywhere.
This is the beauty of life. (Noun)

He made it beautifully. (Adverb)
You are a beautiful person. (Adjective) see what a one word can do if you are aware of these simple things.

9. Add words in your speaking : do you know what is the effective way to add it. Just relate it with your own simpler word. And you will be able to see how soon that word will embarrass you. Like you can take an example of the word inculcate means making others understand.

11. Read all the hoarding and Advertisements at the public place. This will give you good set of words as well as you will learn the spelling of words if you read every day or on regular basis. You would get new words and it will penetrate in your mind.
Actually this one is from my elder brother long time back I was learning typing but I was stuck at the speed of 20 word per minute and no matter how much I tried I was unable to increase my speed then one fine my brother came to me and advised to do typing until your fingers get paining and you won’t believe I started improving. Within a month I touched 38 word per minute with the fine accuracy.
So Once you are set with your basic things then just start doing practice continuously till you get pain in your mouth and very soon you will find yourself comfortable with the language in fact you will become familiar.
Just believe in you and you won’t be betrayed.

I am assuming you know the basics, since you asked this question in English and are ready to understand the answers in English. May be just need knowledge on proper use of grammar, fluency and confidence to use it in your work?
I would suggest you to read English newspapers/magazines/novels daily, preferably read out, while trying to understand its meaning and being conscious about how the sentences and grammar are used.
And start talking in English, however broken it is, at least with your closed ones. Make a rule to talk in only English for few hours daily, and may be reward yourself for following it.
If you watch TV series or movies then watch them in English. Use subtitles and try to get the context. You may have to pause in between to understand better.
Also keep an English dictionary to look up the meaning of the words.
