Almost certainly caused by the aftermarket wheels on the pickup. The wheels are wider than the vehicle is designed for. The inside of the tire / wheel assembly has to be in the same location as the stock assembly to avoid hitting the body when turning. This moves the center of the wide wheel outward. This stresses the wheel studs and also the outer wheel bearing. The inner bearing is much larger than the outer bearing and is designed to carry the vertical load while the smaller outer bearing holds the hub in place. Moving the center of the wheel outwards transfers the vertical load to the outer bearing (which is too small to carry the load) and also exerts and unintended stretch on the wheel studs. Either the outer bearing disintegrates or the studs fail. The rear axle is a different design and most of the time it's the front wheel that fails.

Ambulance wanted to take me to the hospital but I didn't feel any need for that. Until later that evening when I tried to sleep and then I realized I either had some badly bruised ribs. Regardless there's nothing a hospital could have done about that except pain killers and I had those so, yeah, wait 24 hours then tell us.

Truck driver will be sued, and should be. All these folks trying to make their trucks look cool using the cheapest components they can find, see it everyday. They dont want to pay for extra money for quality work and components. This is why impact drivers shouldn't be used to install lug nuts, oil pan bolts, etc. Too many mechanics overuse them or have the wrong settings for the task. All to save a few moments of time.

I put my impact wrench on it's lowest setting and do a final tighten with a torque wrench. Drive about 100 miles, then torque again.

I agree. I have my car serviced at the dealer, and they rotate my tires every 5,000 miles. They use impact wrenches. I have had to replace most of my car’s lug nuts twice as they were stripped and no longer could be tightened.
